Demographic and major changes

CHANGING YOUR PERSONAL Information

Students who are looking to change their Name, Address, or Social Security Number on file with MCCC will be asked to fill out the demographic change form.

The demographic change form gathers students personal information as well as lists the acceptable documents that can be used to make the changes.

If the form is filled out incorrectly, or proper documentation is not submitted, the Registrar's office may deny the request until proper documentation is submitted with the form.

All demographic changes require a valid government issued photo ID, in addition to any other documentation required.

  • Name Changes
    • Official court documents
    • Marriage certificate
    • Updated valid government issued photo ID (license, state ID, passport)
  • Date of Birth Changes
    • Valid government issued photo ID (license, state ID, passport)
    • Birth certificate
  • Address Changes - Requires 2 different documents. You cannot use 2 documents from the same source:
    • Updated valid government issued photo ID (license, state ID)
    • Deed (student's name must be on the deed)
    • Copy of lease (student's name must appear on lease and be signed by witness/notarized)
    • Current Utility Bill (electric/gas, water, cable, phone-landline only)
    • U.S. Postal Service Change of Address confirmation
    • Current Bank Statement
      • (current checking or savings account only - Credit Card, Cashapp, Venmo, or other payment account statements are not accepted)
  • SSN Changes
    • Front and back of original Social Security card
    • If you have an Employment Authorization, we will also need to see the front and back of your Employment Authorization Card
  • Phone Number Change
    • Phone carrier billing statement (student's name and new number must appear on paperwork)
  • Personal Email Change
    • Valid government issued photo ID

 

CHANGING YOUR MAJOR

Students who are looking to change their major during their time at MCCC are required to fill out a change of major form. The change of major form can be obtained by talking with an Advisor (formerly Success Coach) or a Faculty Advisor as you begin the process.

The advisor will walk the student through what changing majors would look like, and what needs to be done to make the change. They will assist you in completing the form and then send it to the Registrar's office for processing.

Any major change that is sent to the Registrar after the add/drop period has ended will be completed for the upcoming semester.

If the form is filled out incorrectly, or is not complete, the Registrar's office may deny the major change request until the form is completed properly.
