CHanging Your Address

Steps to Complete (Need Links Below)

To change your address, complete the steps below that apply to you:

  1. Complete the Employee Phone/Address Change form and e-mail it to Human Resources at hr@mccc.edu
  2. Update your address directly with the Health Insurance Benefitsolver portal
  3. Complete a new W-4 tax form and send it to the Payroll department
  4. Notify AFLAC, if you have a policy with them
  5. If you're enrolled in ABP (the Alternate Benefit Program), notify your provider directly
  6. If you have PERS (the Public Employee Retirement System), you do not need to alert the state of your address change at this time (only update this information with the state when you retire or if you go out on a leave)
  7. If you are in DCRP (the Defined Contribution Retirement Program), update your address with Prudential
