on the untimed test on Accuplacer: 609-586-4800 ext. 3295.

_____ I have had my most recent diagnostic
information sent to the Office of Special Services. (Learning evaluation,
psychological evaluation, medical information)
_____ I have completed the Request
for Academic Modifications and Support Services form and returned
it to the Office of Special Services.
_____ I have completed the Release
of Confidential Information form and returned it to the Office of
Special Services.
_____ I have received my Academic
Accommodation form.
